InovaBehavioral Health is an organization operating in Northern Virginiaand Washington Dc Metropolitan that promotes the total wellness ofindividuals. The organization takes into consideration both body andmind wellness as integrative towards total wellness. The organizationoffers inpatient psychiatry, Inova psychiatric and assessment center(IPAC), as well as psychiatric counseling services. The IPAC is,however, of interest given that all material facts proposed in thispaper were as a result of interviewing Ms. Linda Haake who works inthe demand. Through analyzing the strengths and weakness of thedepartment, I was able to come up with a theory of change.
Theoryof change and problem identification
Theoryof change is outcomes-based and will be used to articulate theassumptions in the problem(Grant, 2012).Grant added that this theory accounts for the how and why theanticipated change comes about. The limitation of an insurance policyis a problem that the organization faces since it does not acceptKaiser Insurance. The organization only accepts Cigna, self-pay, andTricare which require authorization. These standards have seen someof the clients strategically blocked from accessing the organizationvital services. Some of the residents are uninsured and cannot evenaccess the services offered. The assumptions held here are thatclients are more vulnerable to health hazards and risks, they covergreater distance to get affordable health care and they are reluctantto engage in health seeking behaviors. The problem is equally costlywith even some employees seeing instances of double standards withinthe organization`s operation. Culture diversity is also an issue thataffects the organizations especially if they are dealing with MiddleEast clients. However, anticipated changes in telemedicine are likelyto change the fate of some of the challenges the organizationcurrently faces.
Healthinsurance within the United States is crucial since it necessitatesthe provision of medical services in different facilities. Thoughthere are several insurance providers strictly measures are taken byhealth providers in verifying the capability of the insuranceproviders. Neglecting of some of the insurance policies works todisadvantage clients seeking for medical services but living in areaswhere their providers are not recognized.
Culturaldiversity becomes a major issue especially in areas where people of acertain population hold strong beliefs pertaining some issue(Dubay,Aron, Holla, & Peters,2016). Medication normally can be offeredby anybody however, for the people from the Middle East residing inthe United States the case is different. They require that they areaddressed according to their cultural requirement an undertaking thatis hectic given limited workforce from the region in the healthsector.
Multiculturaland social issues
Theorganization relies on insurance providers to offer its services toclients or have diverse insurance agencies. Despite the manyinsurance agencies, the organization relies on one insuranceprovider. These keeps clients who are willing to seek the servicesoffered by Inova a bay since the organization management does notaccommodate them due to existing policies. Clients too who cannotafford insurance policies are locked out of the services offered bythe organization.
Theexistence of client from diverse cultures serves as a potentialhindrance to client satisfaction. Some clients that attend thefacility often require being attended by health care personnel fromtheir own culture. Such acts are common with clients of Middle Eastorigin. This calls for the organization to come up with measures tomeet the client demands. Hiring personnel with knowledge in differentcultures thus becomes an essential factor.
Recommendationto address the problems
Theorganization should institute policies that accommodate all customersof payment. This is because consulting is a complex service whosesuccess revolves around the type of interaction between the actors(Blome, 2014).Inova Behavioral Health will have to bring together the hospitaladministration, state officials and insurance providers to chat onthe best way to meet the needs of the disadvantaged group. Throughaccepting insurance from different organization and pay for servicesterms, it will eventually cater for different clients with diverseincome. Though this may take the time it will ensure that theorganization`s reputation is maintained. Another option will haveInova Behavioral Health initiate their own insurance plan to cut downon the cost per client and the goal will be leveraging the costs tomake health care affordable. To achieve this, the hospitaladministration must liaison with state agencies to acquire a licensethat will see them operate as health care providers and funders.
Telemedicinecan help in dealing with clients within diverse cultures. It is cheapthan having to hire medical personnel from different regions acrossthe globe. The organization only needs to establish a connection witha similar interest in diverse client regions(Blome, 2014).A call center can be established that will link the two organizationstogether in the event a client with a dynamic culture seeks forservices. To establish the connections, however, feasibility studieshave to be carried out that are to be funded by Inova since it has alarger interest compared to the parties it intends to approach(Jordanova& Lievens, 2016). The goal will be to get health services closerto the people by having more physicians available to attend to theclients.
Pre-testand post-test method will be adapted to compare the changes beforeand after the implementation of the recommendations such as startinglocal health insurance plan. At this stage, project leaders will bein position to assess the extent to which the recommendedinterventions have impacted the client and the hospital by way ofstrategic questionnaire
Though,through policies, the recommended actions can be undertaken within agiven time frame. Since changing organizational policies could takemore than a year, I recommend that evaluation is conducted after oneyear of submission of the recommendations. During this time theorganizational management will have to review the differentsuggestion that has put forth. After review, they will then putdifferent measures that are related to the proposed solution.
Whileevaluating the success of the recommendation I will check on whetherthe organization has put into factor changes that I suggested usingthe responses on the questionnaire and checklist. I will consult withboth clients and the management on the insurance policies currentlyoffered by the organization at the time of analysis(Dawson,Watson, Boudreau, & Pitt, 2016). In the case of changes in theinsurance policies offered by the organization and acceptance ofother insurance providers, I will be able to acknowledge that theorganization has implemented changes I recommended.
Checkingon the expansion strategies of the organization is an importantfactor worth taking into consideration. Expansion to other regionsespecially those covering diverse regions is an indication that theorganization has taken into consideration ensuring all its client`sneeds have been met. I will also check on any established callcenters and personnel without American origin working for theorganization.
Exitand follow up
Onceevery suggested idea has been factored I will have to exit from theproject since it will have achieved its purpose. The monitoring afterimplementation will take two months to ensure that the management hascontinued to implement the suggested changes. During this month, Iwill offer advice in case the new plan conflicts organizationalstrategies. After this period, I will exit offering agency servicesrelated to tackling the pertinent issues affecting the organization.I will assign a junior staff in our agency to make follow up ondevelopments within the organization for six months. This willprovide an avenue for checking on whether the agency has helped theorganization solve issues affecting it. It will also work in ensuringthat the organization maintains contact with our agency for futureconsultation services.
Thehospital administration should take initial steps of considering thefeasibility of having hospital-based insurance plan that covers thepatients who are not covered by insurance agencies. Instituteof Medicine IOM (2012) proposes a system that integrates the publichealth and primary care in helping the community meet the goal ofbetter outcomes. IOM identifies the goal of improving populationhealth as deriving its premise from the community effort. As aresult, Irecommend a cost sharing approach where the cost of health care willbe reduced by getting funding from existing clients who are coveredand other funders including the state. An external agency or expertshould be contracted within the period of implementing change toprovide the procedures of implementing the change. This agency shouldbe knowledgeable in health care cover and feasibility studies andwill work in liaison with the hospital management. Implementationshould start immediately to so as to start operation after eightmonths. The goal will be to increase access to affordable health carethrough customized plan. The intervention will include gettingpermission from the state government and addressing other possiblesources of funding to cut down on the cost of care and reach out toaffected segment of the population. In future, data gathering will bedone by way of service rating and customer satisfaction. It will bepossible to assess the number of clients attended and relate withhealth outcomes for this population from hospital driven online datagathering system.
Ouragency values offering value adding services to clients from diversefields, however, it prioritizes consultations related to theprovision of health care services. It has a history of offering valueadding services to clients in the health care services. Through theproposal, the Inova is likely to change the way it offers itsservices to members of the public who rely on the organization.
